WHY DON’T MORE SURGEONS OFFER FLEUR-DE-LIS SURGERY WHEN IT’S CLEARLY NEEDED?

For some massive weight-loss patients, a traditional tummy tuck simply cannot adequately address the amount of horizontal and vertical skin excess.

Yet many surgeons still avoid the Fleur-de-Lis technique because of concerns about the additional scar and the perception that it carries significantly higher complication rates.

We prospectively studied our Fleur-de-Lis patients and evaluated outcomes, complications and reoperation rates. The results challenge some of the assumptions surrounding this operation—and this data has now been submitted for publication.

The goal isn’t to perform a Fleur-de-Lis on everyone. It’s to recognize when it is anatomically the right operation and give patients an informed choice rather than avoiding a necessary procedure simply because of the scar.

IMPLANT WATERFALL DEFORMITY 💧⬇️

This breast implant revision patient came to see me only 2 months after her breast augmentation, unhappy with an early implant waterfall deformity—where the breast tissue descends over an implant positioned beneath the muscle. This can be especially common in patients with more elastic or lax breast tissue.

Unfortunately, her original surgeon was unable to offer her a solution to improve the result.

Revision breast surgery makes up a significant part of my practice, and these cases often require much more than simply exchanging implants. In her case, I converted her from a submuscular augmentation to an above-muscle reconstruction and created a new internal support system using a GalaFLEX mesh wrap/internal bra. 🩺

Another important point: not every implant performs equally well in every pocket. An implant selected for use beneath the muscle may not be the ideal implant when converting to an above-muscle position. Implant selection, pocket control, tissue quality, and internal support all have to work together.

Revision breast surgery is highly individualized—and choosing a surgeon experienced in complex breast revision is critical when the first operation doesn’t go as planned. 🎯
