This patient came to us specifically seeking a breast implant revision, where we converted her implants from subglandular to true subfascial placement, along with a comprehensive mommy makeover that included implant exchange, breast reduction with a mesh internal bra, a tummy tuck with high-definition liposuction, and fat transfer to the hip dips — all performed using our lymphatic-sparing, drainless technique that we created and refined in our practice.
One important educational point: “over-the-muscle” is not a single procedure. There is a meaningful difference between subglandular and subfascial implant placement. Subglandular placement is technically easier and carries a higher risk of capsular contracture, and patients are often told their implants are “over the muscle” without fully understanding which plane was actually used. In this case, her prior implants were subglandular and encapsulated, so we replaced them in a true subfascial plane, providing better soft-tissue support and more predictable long-term outcomes.
