170 lbs down… now it’s time to finish the transformation 🔥

She traveled to St. Louis after losing 170 pounds for a planned 2-stage, full body makeover. Stage 1 was a lower body lift combined with a FDL tummy tuck—because in cases like this, a standard tummy tuck simply isn’t enough.

She had 4-6 inches of horizontal upper abdominal skin that would have been left behind without an FDL… and that’s exactly why we see so many revisions from elsewhere when appropriate procedures are not recommended. If you don’t address that component the first time, you’re redoing the entire tummy tuck.

Why is subfascial breast augmentation so important in patients with loose skin or poor tissue support? 👙

In the wrong patient, placing implants under the muscle can actually worsen a waterfall deformity over time. The muscle pushes the implant upward while the natural breast tissue continues to fall lower — creating an unnatural relationship between the breast and implant. 🌊

In patients with poor skin quality or loose tissues, adding mesh support to an under-the-muscle implant can sometimes become a recipe for the waterfall deformity occurring even faster. 🚫

One of the biggest mistakes in massive weight loss surgery is making scars too short just to “hide” them while leaving behind loose skin. In patients like this, customization matters. Every incision is designed around where the excess actually exists — not around arbitrary scar lengths.

We routinely see revision patients who had an “arm lift” where the scar stopped in the armpit despite significant forearm skin excess… or thigh lifts where the scar never extended past the knee even though loose skin continued far below it. Unfortunately, those patients often end up needing the entire surgery redone because the problem was never fully addressed the first time.

Thinking about arm liposuction? Be careful.

This patient came to me after aggressive lipo caused contour deformities and loose, unsupported skin.

Once this happens, it’s irreversible.
We can improve it—but often only ~50% better, not perfect.

At this point, it requires an arm lift ± fat grafting just to restore shape.

Arm liposuction should almost never be done alone.
If it’s part of an arm lift plan, it has to be done correctly—or it can create permanent deformity.

Same principle everywhere in the body—
we need a layer of fat. Removing too much can be a catastrophic mistake.

Choose carefully.
