This transformation was intentionally performed in two stages.

Stage 1: Breast reduction/lift, with the plan to return later for augmentation and completion of her body contouring.

There’s a reason I don’t perform major cosmetic surgical procedures “awake” in my office.

Cost and convenience should never come before safety, comfort, or surgical quality.

With conscious sedation, patients can still become overly sedated and develop airway or breathing problems. In an accredited operating room, I have an anesthesia professional dedicated to monitoring the patient while I focus entirely on performing the operation.

There’s also the patient experience. Liposuction and other significant procedures can be extremely uncomfortable when performed awake. Pain and movement can make the experience difficult for the patient—and can limit the surgeon’s ability to perform a thorough, precise procedure.

Before choosing a surgeon, ask more than “Are you board certified?” Ask:

Are you board certified specifically in plastic surgery? And do you have hospital privileges to perform the same plastic surgery procedure you’re offering me?

Being allowed to perform some surgery at a hospital is not the same thing.

“Awake” surgery is sometimes marketed as easier, safer, or less expensive. Don’t let marketing or a lower advertised price determine where—or by whom—you have surgery.

Patients who carry most of their excess skin and weight in the lower body are often among the most challenging body contouring cases.

Rather than trying to do everything at once, I frequently take a staged approach.

For this patient, we began with a fleur-de-lis lower body lift combined with buttock and hip reshaping. This procedure removes excess skin circumferentially, elevates the lower body, restores the waistline, and improves the overall contour of the buttocks and hips.

In patients with significant upper abdominal skin excess, a fleur-de-lis incision is often necessary. The vertical component allows removal of excess skin that cannot be addressed with a traditional lower body lift alone, helping prevent persistent upper abdominal folding and creating a smoother overall contour.

This is Stage 1 of her transformation. Once she has fully healed, we’ll move on to thigh lifting to further improve the contour of her lower body.

It’s also important to remember that these results are still early. Swelling will continue to improve, scars will soften and fade over the next 12 months, and her final result will continue to evolve.

She had a tummy tuck a few years ago—but now she wanted MORE.

More dramatic curves. More waist definition. A little more fullness in the buttocks. And additional tightening of the upper abdomen.

For her second-stage transformation, we performed 360° high-definition liposuction to further sculpt the waist and torso, with fat transfer to the buttocks for additional projection and shape.

We also used her own fat to add subtle, natural fullness to the breasts.
